We all know our Health Service is under tremendous pressure. This project is NOT about replacing statutory services.
Often families are anxious about taking the responsibility of their loved-one coming home. Our local communities can offer some neighbourly support and back-up.
Whether it be a weekly phone call or an invite to soup and sweet at the village hall, it all helps build confidence for the patient and their family that will enable them to leave hospital.
Patients will be medically fit to leave hospital and have care arrangements in place where appropriate. It is more about neighbourliness.
