Have you used social prescribing? Do you provide social prescribing services or support? Do you commission it as a GP or health professional If so, they want to hear from you!
The survey will ask you about your experiences of social prescribing, including how it has helped and how it can be improved.
This survey is fully confidential, and it will not be possible to trace what you say back to you. When the survey responses are written up into a report, the findings will be grouped together and will not link what people have said back to the person answering.
