Report: Reimagining care for people living with multiple long-term conditions

Gemma Teal, a Research Fellow, Glasgow School of Art, has recently shared the results of a research project which involved ALLIANCE members in sharing their experiences.

Through their partnership with the Digital Health and Care Institute (DHI), they were asked by The Modern Outpatient Programme at the Scottish Government to explore the future of outpatient care for people living with multiple long-term conditions.

During their scoping work, they identified the goal of policy in moving towards person-centred care but found that the systems and structure of the NHS is designed around specific conditions and clinical specialisms. They wanted to know, how could the health system be designed around the person living with long-term conditions, rather than individual conditions? How would this change their experience of care?
